The Westmont Park District is partnering with Positive Health Solutions (PHS) and the DuPage County Health Department to bring a COVID-19 Vaccination Clinic to the Westmont Park District Community Center, 75 E. Richmond Street, on Thursday, April 29, 11:00 a.m. to 4:00 p.m. The Moderna vaccine will be administered.
MANDATORY ADVANCE SIGN-UP
People interested in receiving the vaccine MUST schedule an appointment using the PHS online sign-up form and bring confirmation with you to your appointment time. Be sure to choose the April 29 date as this is the only time the Westmont Park District Community Center will be open for vaccines. Persons with confirmed appointments should enter the building from the lower level in the back.
If you receive a vaccination on April 29, you should plan on returning to the Community Center on Thursday, May 27. Complete details regarding the second dose will be shared as soon as information is confirmed.
