Board to consider an ordinance approving the following requests for the 2018
Chicago Prostate Center 5K to be held on September 9, 2018:

The Chicago Prostate Center 5K has been a very successful event for several years. This year's request is the same as the past several years.

Police Pension Trustee Reappointment
Board to consider a motion approving Mayor Gunter's reappointment of Thomas Conidi as a member of the Board of Trustees of the Police Pension Fund for a two-year term.
Background of Subject Matter
The Pension Board is comprised of two active officers elected by the members, one retired officer elected by the retirees, and two civilian members appointed by the Mayor. The Mayor appoints one of the two civilian representatives each year.
Additional Background
This year marks Thomas Conidi's 14th anniversary of consecutive service on the Pension Board. He has an MBA from DePaul and has been a financial adviser since 1986. He currently works for Wells Fargo as Vice President - Investment Officer.

Insurance Cooperative Intergovernmental Agreement and By-Laws
Board to consider an ordinance approving the following for the newly formed insurance cooperative named Government Insurance Network (GIN):
A. Intergovernmental Agreement
B. By-Laws
Background of Subject Matter
On March 1, 2018, the Village Board approved a Resolution approving the Village of Westmont to join this new insurance cooperative in which to purchase employee insurance benefits.
Additional Background
Since that time, all members of the insurance cooperative, with the review of legal counsel, have developed and composed these By-Laws and Intergovernmental Agreement.

Government Insurance Network Authorized Representative
Board to consider a Resolution authorizing Catherine Crane, Human Resources Director, as the Authorized Representative and Carolyn Guerrero, Human Resources Generalist, as the Alternate Authorized Representative for the Government Insurance Network (GIN).
Background of Subject Matter
The IGA and By-Laws of the Government Insurance Network Insurance Cooperative stipulates that each member municipality chose in the manner applicable to them, a representative for the insurance cooperative.
Additional Background
An alternative representative may also be designated to serve when the primary is unable to carry out his or her duties.
